Form Builder

The form builder provides a visual interface for creating and customizing interactive forms and surveys with three main panels that work together to design your questions and collect responses.

The Three Builder Panels

Input List

The left panel displays all form fields or questions in order. Add new fields, reorder existing ones by dragging, and select fields for editing. Welcome messages appear at the top, while the thank you message is always last.

Live Preview

The center panel shows exactly how your form or survey will appear to users in real-time. Click on any field in the input list to see its preview, including validation states and interactive elements.

Field Editor

The right panel allows detailed configuration of the selected field. Customize questions, validation rules, options, and appearance settings specific to each input type.

Input Types

Forms and surveys support multiple input types to collect different kinds of information and feedback.

Text Inputs

Single-line text input for brief responses like names, email addresses, or short answers.

Editable Properties:

  • Required field toggle

Choice Inputs

Radio button selection for questions with one correct answer from multiple options.

  • Options list (add/remove/edit individual options)

Rating & Files

Number-based rating scale (default 1-10) with customizable labels for satisfaction surveys.

  • Minimum value (default: 1)
  • Maximum value (default: 10)
  • Lowest value label (default: "Lowest")
  • Highest value label (default: "Highest")

Messages

Introductory message displayed at the beginning of the form or survey.

Choose Appropriate Input Types

Match field types to the information you need - use single choice for categories, multiple choice for features, and numeric scales for ratings.

Keep Forms Focused

Limit forms to essential questions only. Long forms have lower completion rates.
